Yes. The doctor issues an e-zwolnienie exactly as for an employment contract — the type of contract does not affect the document itself. What differs is whether you get paid for the sick days.
Voluntary sickness insurance
On umowa zlecenie sickness insurance is voluntary. ZUS pays the benefit only if your contractor registered you for it and the contribution is being paid. Check this in your PUE ZUS account before you fall ill, not after.
Waiting period
With voluntary insurance the right to benefit begins after 90 days of continuous cover. Earlier periods may count if the break was no longer than 30 days.
Self-employment and B2B
The same rules apply to people running their own business: voluntary insurance and a 90-day waiting period. The sick note itself is issued in the standard way.

Fill in the form, pay and wait for the doctor's call. After the consultation the e-zwolnienie goes to ZUS and a copy arrives by email.
How much a contractor receives
Sickness benefit is 80% of the assessment basis, calculated from average income over the last 12 months on which the sickness contribution was paid.

Log in to PUE ZUS and open the insured person tab — it shows whether your contractor registered you and whether contributions are paid. Check before you fall ill.
Several contracts at once
With several contracts, sickness insurance may cover only one. The basis is then calculated solely from that contract.
Students on a civil contract
Students under 26 on umowa zlecenie are not subject to compulsory ZUS contributions. You will get the sick note, but no benefit.
Contractors from Ukraine
The rules are the same regardless of citizenship: what counts is registration for insurance and paid contributions.
